You and/or your dependents are eligible for coverage if the following guidelines are met:

  • Adults under age 65

  • Dependent children from 15 days to age 25 who:

  1. Have not received consultation or treatment within the last five years for any conditions identified on the application

  2. Are not Pregnant

  3. Have no other hospital or medical coverage in force

  4. Possess a valid Social Security number

  5. Who are foreign citizens that have resided in the U.S. for the last six months.
